Administration instructions

Trisodium Citrate 30% & 46.7% in 5ml

Citra-Lock™ (pH 6.71) is a central venous catheter lock solution, used instead of other locks between dialysis sessions. Side effects are transient and are caused by over filling of the catheter lumen resulting in leakage from the catheter tip. The symptoms are metallic taste (dysgeusia) or tingling in the fingers (paresthesia). These potential symptoms need explaining to the patient prior to administration. If symptoms occur, reduce the volume of the next lock by 0.1ml per lumen (see notes). Citra-Lock™ should be administered slowly over 5-10 seconds.


Pre-dialysis (maintain asepsis)

  • Prior to initiation of dialysis Citra-Lock™ should be aspirated and discarded and catheter flow assessed according to local policy.
  • If unable to aspirate Citra-Lock™ use 10ml saline 0.9% and flush the catheter lock solution into the patient over 20-30 seconds. If both lumens require flushing leave several minutes between each flush. Explain to the patient they may experience the symptoms described above.

Post-dialysis (maintain asepsis)

  • Flush each catheter lumen with 0.9% saline as per local policy.
  • Using a 2,5 ml syringe draw up the exact volume of Citra-Lock™ for each lumen and inject slowly over

5-10 seconds.

This volume may be reduced if symptoms were experienced in previous administration (see notes).

 

Notes

If biofilm or thrombus is present within the lumen the volume will be reduced and the estimated locking volume may over-fill the catheter causing some locking solution to enter the blood stream, this may cause the symptoms described above. Reduce the volume of the locking solution by 0.1ml for the next treatment and repeat until the symptoms disappear. Reducing the catheter lock solution by more than 0.1ml per lumen may increase the likelihood of intraluminal clotting by leaving space for blood to re-enter the catheter tip. The external catheter limbs may become discoloured when locked with Citra-Lock™, this is not significant.
